Publish (Deploy) Web application and Database Project using Visual Studio

If I am publishing my website using visual studio which includes a local database, do I need to create another database on the host server? Or when publishing will the database automatically is created on the server?

I just want to have a clear understating on this. Please can anyone guide me through with a step-by-step guide?
