Multiple Input text value in sweetAlert and show value in textbox after click ok in asp.net c#

I want to capture REMARK-1 and REMARK-2 . when I click Save button sweet alter should ask value or REMARK-1 and in second line REMARK-2 when i click on it should display in Textbox1 and textbox2.
