problem in fileupload

HI 

I use this link for put file upload control in my page

i download file and it worked correctly but when i use that code in my website it didn't show fileupload button 

here fileupload control have two part 1-white div that show selected image url  2-button Text="select file" both of them have tooltip

when i use code in my page it just show white div  it didn't show button but i can see button's tooltip ...
